Instructions for Everyone: 

  1. Pay all membership fees and screenshot receipt that includes name & membership acquired (**for P1s, you only need to make an account for APhA & CPhA - your 1st year is paid for by Chapman) 
  2. Submit your screenshots to your respective Class Google Form

NOTES for: 

P1s:
•    No payment is needed for APhA National Membership or CPhA State Membership so please DO NOT PURCHASE IT through the APhA or CPhA website because you will not be refunded
•    Upload a screenshot of your created account in your Google Form
•    Your ONLY payment is the chapter fee within this Marketplace link
•    Upload a screenshot of your receipt to your Google Form
 
P2s:
•    Renewal of your APhA Membership will be on the APhA website
•    If you choose to renew your CPhA membership, it will be on the CPhA website
•    Payment of chapter fees is done within this Marketplace link
•    Please upload all screenshots of receipts in your respective Google Form
 
P3s:
•    There is the option to do either 1 year APhA National Membership or 2 year Transitioning APhA National Membership (that includes post-graduation resources)
•    1 year APhA National Membership requires payment through the Marketplace link AND payment of chapter fees through the Marketplace link
•    2 year Transitioning APhA National Membership will be paid through the APhA website AND payment of chapter fees through this Marketplace link
•    If you choose to renew your CPhA membership, it will be on the CPhA website
•    Please upload all screenshots of receipts in your respective Google Form
