2025 UConn 4-H Dog Clinic

The 2025 UConn 4-H Dog Clinic will be held on Saturday, May 3, 2025 at the Tolland Agricultural Center, 24 Hyde Ave, Vernon, CT, 06066. Clinic is held from 9:00 am - 1:00 pm, Check in starts at 8:00 am. The registration deadline is Friday, April 25, 2025. Space is limited to 35 participants and their dog.

REQUIRED DOCUMENTS 
The following documents must be emailed (subject line dog clinic) to tolland@uconn.edu by April 25, 2025 or mailed to Tolland County Extension, 24 Hyde Ave, Vernon, CT 06066, postmarked by April 25, 2025. Please ensure the following documents are submitted: 

1. Proof of current rabies vaccination 

1. Proof of current Distemper & Parvovirus vaccination 
   (Titers for Distemper & Parvovirus done within one year of the event are acceptable.) Lyme disease and Kennel Cough vaccinations are recommended but not required 

1. 4-H'er Health Form (if the youth participant is not accompanied by a parent/guardian, These are available at https://4-h.extension.uconn.edu/forms/.) 

Lunch option can be added on to registration. Boxed lunch included a 6" sandwich with lettuce, tomato, cheese & condiments (on the side), as well as a cookie and bag of chips.
