Course Overview:
In spite of all our best efforts, accidents and incidents occasionally occur. These unplanned or undesired events can adversely affect a company’s work operations and may include work-related injuries, occupational illnesses, property damage, spills and fires. Accident investigations are necessary to identify accident causation and ensure the effective implementation of corrective actions. A minor incident or near-miss incident also requires a thorough investigation as this may reveal significant potential for a serious future incident should the conditions be left uncorrected.
The Mary Kay O’Connor Process Safety Center Extension in Qatar and GEXCON present this 5-day course on: “Detailed incident Investigation”. This course will provide the attendees with an overview and basic knowledge of incident investigation; theory, approach, investigation process (e.g. setting up investigation team, mandate), methods and tools, and reporting of incidents (including follow-up). In addition, this course will give you a broad understanding of the accident investigation process, looking at the benefits of accident prevention and putting the emphasis on practical training exercises and real-life case studies. Participants will also learn how to uncover the who, what, why, when and how of each incident, and how to analyze data to prevent injuries, property damage and financial losses.

Learning Outcome
This course will provide the knowledge and skills required to carry out effective incident and accident investigations.

Who should attend
The course has been designed for managers and supervisors with responsibility for reporting and investigating accidents within their work areas.

Course Instructor
Dr. Scott Davis is the President at GexCon US and specializes in the engineering analysis and testing of combustion, thermal, and fluid processes. Dr. Davis received his Ph.D. and M.S. degree in Mechanical and Aerospace Engineering from Princeton University. Dr. Davis is responsible for fire and explosion related activities, which include post-incident investigative work, worldwide training and experimentation, as well as performing risk assessments and safety studies for Oil & gas installations, petrochemical facilities, and various other industries. These studies include explosion risk assessment, blast and venting analyses, assessment of combustible dust explosions, toxic/flammable gas releases and dispersion, hydrogen safety, ventilation, detector placement, and carbon monoxide dispersion. He has investigated hundreds of fire and explosions, and previously served on the technical committee for NFPA 921. Dr. Davis is a member of GexCon’s docents group, which develops and delivers worldwide industrial seminars to owners, operators, safety engineers, and regulatory agencies, on the hazards associated with gas explosions, dust explosions and LNG.
