To reserve your spot, complete this form and provide credit card payment.

Please Note: If your child has other activities and is not available for the entire time above, please select a different day for tutoring. Tutors MUST tutor within the designated time slot. Your child’s attendance and full participation during sessions is vital in order to maximize the academic growth of your child and the preparation of our pre-service teachers.
Children should join from an environment with few distractions using a computer, laptop, or tablet.
Pencil and paper may also be needed. Students who miss
two
tutoring sessions may be dropped from the program for the semester, and the tutoring fee will
NOT
be refunded. Students with excessive absences may not be allowed to return for tutoring in future semesters.
Sessions are recorded throughout the semester for current and/or research, diagnostic, and/or instructional purposes. To reach optimum efficiency, assessments and evaluations may be administered to determine initial placement, identification of strengths, needs, and progress. Whenever student records are used in an instructional situation, confidentiality is maintained in compliance with the Educational Rights and Privacy Act of 1974 (PL 93-380). Whenever student records are used for research purposes, confidentiality is maintained in compliance with the OSU Institutional Review Board and Title 45 Code of Federal Regulations Part 46 (45 CFR 46 "Basic HHS Policy for the Protection of Human Subjects”).
