Flex Private Swim Lessons-6 Pack

General Policies
Non-transferable Sessions: Swim Lesson packages may not be transferred from one participant to another. Sessions must be used by the original client.

Attire: Participants a required to wear appropriate swim attire. Street clothing such as cut-offs, sweatpants, basketball shorts, and long sleeve shirts are prohibited. Sports bras may be worn if they are a dark color and provide appropriate coverage. Jewelry is subject to lifeguard approval.

Cancellation/Refund Policy
Refunds are not provided for swim lesson packages except in extenuating circumstances. Make up lessons will not be provided if a session is missed.
 

Individual Participant Waiver and Release Agreement

Activity: Swim Lessons

In consideration for the right to participate in the above described activity (“Activity”), I hereby agree to this waiver and release.

I recognize that the Activity may have a risk of personal injury and/or damage to my property. This includes: transient light headaches, fainting, abnormal blood pressure, chest discomfort, muscle cramps, nausea, stroke, heart attack, and/or death. I hereby freely assume all risks which may be associated with or result from participating in the Activity including, but not limited to: travel to and from, instruction, participation, and competition.

I further agree to release the State of Utah, Weber State University (WSU), their officers, employees, agents, contractors and volunteers (“Releasees”) from any and all liability, claims, demands, actions, loss, claim, damage, injury, illness, or harm (“Claims”) to me or my child of any kind or nature arising out of participation in the Activity including where Claims occur due to the negligence of Releasees.
